1. Make a copy of this document for your records before mailing.

2. Mail this notice to the defendant via certified mail and ask for a Return Receipt Request.

3. Affiant must include their contact information when mailing this document to the Sheriff's Office so that we can contact you if we have additional questions.

4. Once you receive the Return Receipt back from the post office, mail a copy of this notice along with a copy of the return receipt to:
